Submission Instructions
For Application Materials
(note: you may download a PDF file of these instructions from the FORMS PAGE)
To apply to become a contributing author, you should read all of the forms on the forms page, but you must only complete and submit the following forms. You must send the entire form, except for the Author Agreement as indicated below.
1. Contributing Author Application & Profile
2. Contributing Author Agreement (you may send just the first and last pages that contain your name and signature)
3. Schedule A for Individual Authors (or Joint Authors if you're partnering with another contributing author)
Methods of Submission
• Scan documents and send as email PDF attachment to dan@collpress.com (preferred method)
• Fax documents to Collaborative Press at 402-218-2006
• If paying by check, please send documents via email or fax, and make check payable and mail to:
